GREENVILLE, S.C. — WYFF News 4’s Buyer Beware reporter Tim Waller is warning people to be on the lookout for a certain tax form that could get them in trouble.

The form is known as W-9095. It appears to be just another tax form, but it does not come from the Internal Revenue Service.

Buyer Beware Fake IRS Forms

Greenville certified public accountant Renita Owens said it looks like something the IRS would send, but it asks for too much personal information.

“It has Department of the Treasury Internal Revenue Service written on it … In this section, it wants to know the date and amount of the last deposit,” Owens said. “They want to know your password and your PIN number, and of course your Social Security number.”
